Constance Care Management is staffed by owner Bridget Jardas, LPN, CMC. Backed by many years of combined experience, we serve Monroe County and the bordering areas of Ontario County.
A Message From Connie
What's behind our name
I put a great deal of thought into naming my Aging Life Care™ practice. The name “Constance Care Management” conveys a message about how I like to serve my clients – with constancy and with care. My name, Constance, also carries a special meaning for me. I was named after my maternal grandfather, Constant Southworth. I was born on the sixth anniversary of his death. He died suddenly of a heart attack at the age of 56. I never had the privilege of knowing my grandfather personally, but know from the stories my mother has shared that he was a kind and loving man who worked hard to support his family. I know he would have been a wonderful grandfather to me and I feel honored to carry his name. It seems fitting that a service that strives to create the best possible quality of life for clients as they age, pays homage to a man who never had the opportunity to grow old, my grandfather, Constant Southworth.
